All Health Forms are contained in the 2009 Pilgrim Lodge Parent & Camper guide.

Pilgrim Lodge Health Forms

Before you download the forms please read this:

Send Health Forms at least two weeks before the start of camp to:

Children & Youth Attending Camp (other than Family Camps)
Form 1
(three pages) completed by a parent

Form 2 (one page) top portion completed by a parent, remainder by your physician

Form 3 (one page) only necessary if the child must carry medication on his or her person. (ie” epi-pens or inhalers) completed by parent and physician

If you submitted a form last year, signed by a physician in 2008, you do not need to re-submit a form this year, however, you will have to update and initial last year's form on arrival. If you are not bringing your child to camp yourself, be sure to complete the attached Form 1 and send it along with your camper. Call or email if you’re unsure about your health form status (724-3200 or karen@pilgrimlodge.org).

Adults Attending Adult Camps: Ignore forms 1-3.   Fill out form 4 (one page) and either mail it in before hand or bring it with you.  We’ll have some on hand when you arrive but it’s best to get this out of the way beforehand.

Family and Grandparents Camps:

Family  camp: no forms needed unless you are bringing a minor that is unaccompanied by a parent or legal guardian.  If you are bringing a minor that is unaccompanied by a parent or legal guardian, follow the steps above for “children attending camp” for those children.

Grandparents Camps:

Children: Follow directions above for “children attending camp.”  Parents, not grandparents, must sign the forms.

Adults: complete and submit form 4 only please.

 A Day at Camp: no forms needed unless you are bringing a minor that is unaccompanied by a parent or legal guardian.  If you are bringing a minor that is unaccompanied by a parent or legal guardian, have the parent or legal guardian complete and sign form 1, and if necessary form 3.  Ignore forms 2 & 4
