For those attending family camps or Grandparents & Grandchildren Camps.
Health Forms
Minors attending camp at Pilgrim Lodge without a legal guardian need forms 1 and 2. Form 3 is also needed if they carry an epi-pen, inhaler, or other emergency medication. Forms 2 and 3 need a doctor’s signature. The Health and Emergency Information Form is for Adult Campers and minor-aged campers who attend with their parent or guardian.
Health Form 1
Minors attending an event at Pilgrim Lodge without a legal guardian need this form.
We encourage parents to fill this out online as part of the registration process.
Health Form 2
Minors attending an event at Pilgrim Lodge without a legal guardian need this form. It needs to be signed by your doctor.
Health Form 3
Only minors who carry an epi-pen, inhaler, or other emergency medication attending an event at Pilgrim Lodge without a legal guardian need this form. It needs to be signed by your doctor.
Health and Emergency Information for Adults and Families
All adults need to fill out this form. This form is also for minor-aged campers who attend a session with their parent or guardian. It does not need to be signed by a doctor.
